The traveler receives two facilities: shortening the four-rak'a prayers to two, and joining two prayers together. The Companions lived these facilities with the Prophet (peace and blessings of Allah be upon him): Ibn Mas'ud said: "I prayed two rak'as with the Prophet, then two with Abu Bakr, then two with 'Umar, then the paths parted; would that my share of four were two accepted rak'as" (al-Bukhari 1574, Muslim 695). Anas reports that the Prophet shortened the prayer all the way to Mecca and resided there ten days praying two rak'as (Muslim 693).
The Hanafis and one Maliki position hold that the traveler's four-rak'a prayer is, in itself, a two-rak'a prayer: the ease is fixed, not an option, relying on 'Aisha's words: Allah prescribed the prayer two rak'as by two, in residence as in travel, then the travel prayer was kept that way (reported in the two Sahihs). The Shafi'is and Hanbalis see in it a permission (rukhsa) granted to the traveler.
The Malikis, Shafi'is and Hanbalis set the minimum distance at about 86 kilometers: the journey of a moderate day and night, or the equivalent of four "barid" (48 miles). Ibn 'Umar and Ibn 'Abbas shortened at that distance (al-Bukhari 1089, Muslim 1338). Among the Hanafis, the reference measure is the journey of several days of walking or caravan travel. Anas also reports that the Prophet prayed two rak'as from three miles of travel (Muslim 691): Ibn Hajar considers this hadith the most sound and explicit on the subject.
For rain, mud or cold, the schools vary. The Shafi'is refuse joining for mere mud, since this existed in the Prophet's time without him joining for it. The Hanbalis, in the soundest view among them, allow joining for strong wind on a cold dark night, citing the Prophet's call on rainy nights: "pray in your dwellings" (al-Bukhari 606, Muslim 697).
When traveling, settle your position before departing and stay consistent throughout the trip: those who treat shortening as a facility keep the choice; those who see it as fixed pray two rak'as without regret. Keep joining for a real need: rain, waiting, genuine fatigue.
